Critical Incident Report

This reporting form is intended to be used periodically by the cooperating teacher or university supervisor to document any critical incident that occurs during a student teaching placement. The form can be used to highlight exemplary student teacher performance and/or problem behavior. For example, a cooperating teacher could complete a critical incident report if a student teacher handled a difficult situation with a parent particularly well (positive incident) and/or report a student teacher who was overheard making an inappropriate comment about a child in the faculty room (negative incident). Basically, this report should be used to report any critical incident that could positively or negatively influence a student teacher’s standing in a particular placement.
